摘要
본고는 불교의학의 질병관을 논한다. 불교의학은 고대 인도의학에 불교적인 질병 이해와 치료법이 더해진 것이다. 불교의학을 전반적으로 다룬 선행연구들은 신체적 질병과 정신적 질병, 그것들의 치료만을 논한다. 그러나 불교의학에서 다루는 질병의 범주에는 실존적 질병도 포함되어야 하며, 그것을 다른 질병들에 우선하는 근본적인 질병으로 이해해야 한다. 실존적 질병을 불교의학의 근간에 두지 않고 불교문헌과 승려, 승단의 활동에서 나타나는 신체적 질병, 정신적 질병만을 떼어내서 논하고 활용할 때, 그것은 자칫 불교에서 가져왔으나 비불교적인 질병 이해와 실천이 될 수 있기 때문이다. 이러한 이유에서 본고는 불교의학의 질병관을 실존적 질병, 신체적 질병, 정신적 질병으로 나누어 논한다. 실존적 질병은 우리의 삶과 죽음 전체, 그 무상함과 유한함 및 이에 대한 인식으로부터 오는 고통으로서, 둑카(苦, duḥkha)에 해당하는 질병이다. 신체적 질병은 신체를 이루는 네 가지 요소인 땅, 물, 불, 바람의 조화가 깨졌을 때 일어나는 질병이다. 정신적 질병은 두 종류로 나눌 수 있는데, 첫째는 범부 일반이 가지고 있는 번뇌이고, 둘째는 광인의 정신질환이다.
In this paper, I discuss the concept of disease in Buddhist medicine. Buddhist medicine involves a Buddhist understanding of diseases and treatments based on ancient Indian medicine. Previous studies that discussed Buddhist medicine only dealt with physical disease and mental illness. However, existential disease and its treatment should be included in Buddhist medicine and understood as a fundamental disease that takes precedence over other diseases. Without discussing existential disease, Buddhist medicine can be non-Buddhist. I Therefore discuss the concept of disease in Buddhist medicine by dividing it into existential disease, physical disease, and mental illness. An existential disease is one that corresponds to a “duḥkha”, a pain that comes from the impermanence and finiteness. Physical disease is one that occurs when the harmony of the four elements of the body — earth, water, fire, and wind — is broken. Mental illness can be divided into two types: the defilements(“kleśa”) that all people have and diagnosable mental illness.
